Please fill out the following form and our office will get in contact with you regarding your questions. In-person clinic appointments can be scheduled with our office once you have registered for the Foundations in Functional Health 3-month program. 

In clinic assessment and treatment packages begin at $2200 for 3 months of treatment including 1- 90 minute evaluation and 6 1-hour follow-up hands-on sessions.

Intensive packages available for both local and out of town clients and patients. Twice/day treatments will be scheduled over over 2-3 days. Please contact the office for pricing.
